1. Accidents, Illness and Emergency
I understand that in the event of sickness,
an accident or any form of emergency, Peaks Palm
Academy will act on my behalf until such a time
as I can be present.
I understand that every effort will be made to
contact me as soon as any illness or injury is
identified.
2. Medication / Calpol Authority
In the event of my child having a high temperature,
I consent to my child being given Calpol at the
school by a qualified member of staff, following
procedures laid down by the school.
3. Outing Authorization
Occasionally, as part of learning, children may
be taken outside the school premises to local
facilities according to the demands of particular
learning areas.
4. Photo and Video Consent
Peaks Palm Academy may sometimes take photographs
or video footage of children while taking part in
school activities.
5. Data Protection
Any information provided to Peaks Palm Academy
will be kept secure and treated in confidence.
6. Faith Consent
Peaks Palm Academy is a Christian faith-based
institution which believes in Bible teachings
as a major source of instruction for a holistic
learning experience.
7. School Fees
All fees shall be reviewed every year and parents
shall be given one month's notice of any alterations.
8. Parent / Guardian Declaration
I confirm that the information provided in this
admission form is accurate and that I have read
and understood the relevant school policies and
consent requirements.
